I remember a long while back when you became fat you would burst from your clothing and become naked. This feature added a good degree of clean humor to the game when some fatty fucker overate a shit ton and now police are chasing a super fast obesity man down the hallway trying to force clothes on him. However this was a problem due to the fact that fat diseases, and accidentally over eating forced players to gob down diet pills or go on long jogs.
What I am proposing is that fat sprites are re-introduced with one new mechanic. That there be a kind of third-threshold of eating cheesy honkers that is clearly intentionally making oneself fat. So past "vigorous" there is "fatty fucker status" or something that is only reached by going way, way over the top. This way people's jumpsuits are not taken off due to lack of sprites, unless someone intentionally becomes a fatass.

The issue is that we would have to resprite every jumpsuit to have a normal sprite and a fat sprite. Nobody is willing to do that much work.
Except its been suggested many times that we simply have fat people burst from their clothing and can only fit in plus-sized jumpsuits, thus reducing the spriting needs from every jumpsuit in the game to four or five.
Not that it will happen anyway. I have a sneaking suspicion that someone in the coding team is a fucking beached whale because every time fat sprites come up the thread usually gets locked with a resounding "no'.

If the resulting code from the upcoming dismemberment and mutation rewrites makes this not a maintainability problem, then maybe. Until then the real problem is, well, mutations and other mob-based update_icons() special cases.

Couldn't you just make the fartyization thing find the next best jumpsuit? The colored non-job specific sprites all have fat sprites. Captain's becomes just a regular blue jumpsuit, sec gets red, engi yellow, etc. You can even add flavor text about your Jumpsuit's seams distorting.
