Space-quest style Death Messages.
This is a simple yet funny idea.
If you get acted upon by something liable to kill you you will see a popup window death message based on said thing accompanied by imagery, a jaunty death jingle and maybe we can convince some particularly rich voiced spacemen to provide voiceovers.
I'll go one better than outline concepts and go so far as to suggest a code approach
The player mob should gain a new variable, a simple string, on the mobs death this string is run against a list, this list contains a series of structures/arrays/whathaveyou that contain the data necessary for a death message. For each match add the data structure to a list and pick one at random.
ID, death-cause, text, image, voiceover.
ID is a unique identifier, death-cause is the string searched for, text is the message, image is a funky graphic representing your demise and voiceover is any audio voiceovers you might have gotten.
This data is fed to a new popup window that will be shown to the now dead player.
Example, you set the base item class to have a death-cause of blank. To the "onhit" proc (or whatever gets called when you apply item to mob) add a "if (death-cause) player.death-cause = item.death-cause"
On player death "if(death-cause && prefs.deathmsg) run-death-message(player, death-message)"
You would then set the death-cause of the toolbox to "toolbox" and if the player dies with the cause "toolbox" then they will see one of any of the death messages with the death-cause of "toolbox"
By carefully adding sources of death-message to various damaging procs you'll end up with a player seeing a death message based on thier cause of death most of the time.

Some big brain bonus objectives would be to add a button for ghosts to review thier death window, or see the death windows of other mobs. Add some extra info to the death window like time of death, damage taken etc.. Make it so that one deathmessage can have multiple causes that would be relevant and have the proc find them.

While a fun idea, I can't see voice acting ever being a part of this. There are just too many ways to die, and even if lines were recorded for all of them, the day will come where our smooth-voiced space narrator drops off the face of the earth, leaving us with no one to narrate new deaths.

In theory you could have multiple VA's and have some deaths not voice at all, inconsistent yes but better than no voice IMO.
