I could make a scabaras disease that tears apart the victim.
The person that purchases it gets a one off syringe which changes their body to a scarabite, but in return loses their humanity.
A scarabite has benefits over humans, and allows them to be a carrier for scabaras without themselves being affected.
People afflicted will take damage as the scarabs pop out of their system and attack them or others.
Scarabites are immune to the damaging effects, and will not be attacked.

That sounds interesting but make the scarabs literally reskinned bees or else they'll be overpowered as fuck. Also make them do little to no damage on popping out. Beesease basically, but more common.
