Was memeing back and forth with a mime through subtle messages. I didn't realize he was about to go crit so I BSA'd him which gibbed him in the middle of medbay. Control + shift + Left clicked his ghost to immediately bring him back, but I still felt bad about it.
The worst thing I think I've done involved Imszx. I failed to check his objectives when he was an antag so I thought it was the standard traitor Lexia murderbones the entire station round. Turns out he had the hijack objective. I dropped a 6 man ERT team on him. He proceeds to murder every single one of them and complete the hijack. I felt terrible when I noticed the hijack objective and I didn't want to delete the ERT. I've been a bit more careful about ERT deployment since then.
Opened the random event trigger menu looking for something to add some mild spice to the round, tabbed out of the window to look at a discord ping, tabbed back in by clicking on the window.
My mouse happened to land on the "Meteor Wave: Catastrophic" button. On Northstar. With the rad nebula station trait. With nobody playing engineering. Less than 15 minutes into the round.
I tried to mitigate the damages by using the orbit menu and manually deleting every meteor I got sent to and deleting the event from the subsystem, but it did enough damage to lock up atmos and make several parts of the station uninhabitable even after repairs. One misclick turned what could have been a cozy 2 hour manuel round into an incredibly brief CBT fest for everyone involved.
Once a Captain ahelped that they had to leave, so, in order to place a Captain with a different name to secure the player's identity, I deleted the old Captain and spawned a new one with Cap gear.
It turns out the Captain was a Heretic's target... We couldn't easily change targets at that time without heavy VV shenanigans... The poor guy couldn't ascend because of that... Sorry, random Heretic!!

Several times spawning ghosts into the thunderdome when they still had revivable corpses. Always awkward. Always frantically putting them back. Occasionally they’d die and come back to ask to be put in the Christmas Tree Wars again anyway.

I once tried to move a wisdom cow out of the bar on a low pop round so that someone might interact with it or listen to its ramblings. Reused a pod window I already had open that I'd set to missile mode / devastate 5 or something, wiped the cow and the bar out nice and clean. No-one nearby at the time but was amused watching the crew be confused and speculate over what had happened. Sadly not the only time I'd reuse a pod window though never to this effect. Kinda got into the habit of just closing them once I've done my drop now. (Someone fix pod presets, it's been years)
I also spawned in TD once and linged up to test something for an ahelp, and then accidentally de-antagged the player instead of myself. Gave em a token and re-linged em since I probably screwed some progress measures. Only antag token I've ever given I think.
